We do not require forms to be completed prior to your first psychotherapy appointment. However, in the interests of making the most of your time during your initial session, you may download, print and bring your completed patient registration forms with you to your appointment. Please let us know if you have any questions about these forms or need assistance in completing them. On occasion, when working with families and teenagers, it is useful to complete a family contract specifying behavioral expectations, rules, privileges and consequences. This form is usually completed in the context of ongoing family counseling, where parents and their adolescent are negotiating house rules.
